Advance Care Planning

It’s never too early to begin advance care planning, and taking these steps now can bring clarity and confidence to you and the people who love you. As a practitioner trained through The Dying Year, I can guide you through choosing a healthcare representative, exploring your end-of-life wishes, and having the hard but essential conversations that ensure your voice is heard. This work isn’t about fear—it’s about empowerment, making sure everyone has a say in what happens to their own life, values, and care.

Advance Care Planning Services:

  • Guidance using the nationally known Five Wishes program
  • Facilitated conversations including you and your healthcare representative(s)
  • Materials and education regarding CPR and other advance care situations
